Meet the Team!
Jeff
​We are the Cooks! With 6 kids and a dog we live the typical crazy family life! With carpooling to sports practices, cleaning up messes, camping, going on family walks, laughing, crying and everything in-between we stay pretty busy.
​
I graduated from Southern California College of Optometry in 2013 and then we moved to Bullhead City, AZ for my first job at an ophthalmology practice called "Western Medical Eye Center". During my years there I had extensive exposure to and became comfortable with any and all medical eye conditions. And let me tell you, they see it all at WME! The doctors there are great and I was fortunate to have this educational experience.

In 2016 we moved to Provo, UT and though we do miss all our friends in Arizona we do love it here! I started working at the Orem Target part time and soon became what I call a "traveling optometrist." Somehow word caught on that I was available to cover shifts and in 2021 alone I worked in 13 different offices, everything from corporate to private practice.

With the education and experience I had obtained I decided it was time to venture out on my own and created "Cook Family Eyecare". Having worked at so many offices made knowing exactly what I wanted to do when it came to practicing (and didn't want to do) really easy. ​I wanted to create a comfortable, transparent, low-pressure environment where people felt heard and felt like they had a friend in the eye world who cared about their eyes. I genuinely enjoy getting to know each of my patients and helping them see their best and do what I can to help them prevent devastating eye disease through education and monitoring.

Alex
Alex was our first employee, she came to Cook Family Eyecare with a vast knowledge of the optical and eyecare industry that was acquired over 10+ years in optical, 3.5 years at Walmart Vision Center and 4.5 years at Eyeglass World - where she was the Assistant General Manager over 11 Opticians and Optical Technicians and has been a huge asset to the practice since day one.
Not only can she educate patients on the ins-and-outs of all the different lens materials,coatings and options, but she also has a knack for selecting the perfect frame for anyone looking. When it comes to repairing and adjusting frames she is somewhat of a magician!
Alex always has each patient's best interest at heart and does all she can to make sure patients are educated and happy with their selection. She also wears many hats as she does billing, shipping, receiving, frame and style selections for the practice and helps out in all other areas in between! We are lucky to have her with us!
Optical Manager / Optician
Melody
If you've ever made a call to or walked into Cook Family Eyecare chances are you have spoken to Melody! Melody has a way of befriending anyone and everyone so effortlessly and has played a pivotal role since she joined the practice.
​
Melody has a history of great customer service from working at Walmart as a student (where she met her husband) to being a dental assistant to then having five children (which is next level customer service). Having a large family also explains why Melody is so good at multi-tasking and has a can-do and can-learn anything attitude. She is a person who sees a need and fills it, and her help has alleviated a lot of stresses!
Office Manager
Michelle
The moment we met Michelle at her annual eye exam we knew we needed her to be a part of the Cook Family Eyecare team and to our surprise she accepted the job offer! Michelle has previously worked at Logan Optical and Standard Optical where she gained experience with all things optical.
We have been impressed with Michelle's ability to put patients at ease, making them feel seen and comfortable with her kind and caring demeanor. Michelle likes baking, working out, animals and musical theater. She hosted the first annual Cook Family Eyecare barbeque at her house and it was the party of the century!
Receptionist / Technician
Jadyn
Jadyn is currently a student at Utah Valley University, working toward a future in forensic science. She thrives in an academic environment, especially when it comes to medical science, and loves diving deeper into the world of healthcare. She spent two years training as an EMT, which sparked her curiosity and passion about healthcare and patient well-being.
​
When she’s not studying, Jadyn enjoys spending her free time with family, learning a new skill, or creating friendships with new people. Fluent in Mandarin Chinese and always picking up more languages, she’s excited to connect with people from all cultures and walks of life. She has a knack for making others feel at ease and loves creating a welcoming, friendly atmosphere.

With her creative spirit and constant drive to learn, Jadyn enjoys being part of a team where she can contribute and grow. Whether she’s chatting it up with patients or working behind the scenes, her positive energy and enthusiasm for everyone shines through in everything she does.
